Home health occupational therapist in Carlinville, IL providing in-home OT services to patients as prescribed by the attending physician, plus OT consultation to staff and physicians. Perform OT assessments, set short- and long-term goals, implement treatment programs, instruct patients and families in therapeutic and self-care activities, train in orthotic/prosthetic equipment, conduct re-evaluations, maintain clinical notes, report progress to the physician, and consult with the home health team. Schedule is Monday–Friday 8:00a–5:00p plus one weekend per month and two holidays per year; typically sees 5–6 patients per day; local travel.

Requirements

Bachelor's degree or higher · 1+ years Occupational Medicine · 2+ years Social Work · Valid IL OTR license · Valid driver's license · AHA BLS or ACLS or PALS
